How to Access

Homelessness support can be accessed through the Yarra Community Housing MetroWest Initial Assessment and Planning (IAP) service

It is one of the Western metropolitan region’s main response services to people who are homeless or in housing crisis. It is for families living in Hobsons Bay, Maribyrnong, Melbourne, Melton, Moonee Valley, and Wyndham.

For those living in the Brimbank and Melton Local Government Areas, the response service for people who are homeless or in housing crisis is the SASHS Network who offer an Access Point.

For those living in the Goulburn Valley, the response service for people who are homeless or in housing crisis is the Rural Housing Network.

About the service

Our specialist homelessness service provides short to medium term housing for vulnerable pregnant women, and families with young children.

Casework is usually undertaken in the home and is focused on transitional support to move you from being in crisis situation to transitional housing and into long term accommodation.

Using the Outcome Star tool, Homelessness Star, we work with you so that you can assess and review your capacity in the following domains:

  • Motivation and taking reponsibility,
  • Self-care and living skills,
  • Managing money and personal administration,
  • Social networks and relationships,
  • Drug and alcohol misuse,
  • Physical health,
  • Emotional and mental health, and
  • Meaningful use of time.

The program is funded as Transitional Support by the Victorian Government.
